Musical Background Survey

What is this survey for? 

This is a survey of individuals that belong to Making Music member groups. Its focus is on the musical background of those individuals and is designed to help groups learn more about their membership. Making Music will also use the data from this survey to find out more about our membership and their musical background. All the information is completely anonymous, and you will not be identifiable by Making Music or your own group.

A couple of the questions will not be relevant for your group to know more about its members, but they will be useful for Making Music when we look at the whole set of data.
